Forms	Submission Process
Veterans Treatment Court Application	Submit application packet to the Administrative Office of the Courts/ Veterans Treatment Court via email to: VTCapp@fljud13.org Questions can be addressed to the VTC Office at (813) 307-4475.
Veterans Treatment Court Consent Form	If utilizing a VA location outside of Hillsborough County, fill out highlighted areas and submit to VTCapp@fljud13.org
Mentor Application Mentor Background Check Form	Submit an inquiry on the Contact Us section of the mentor website, located here. Once contact is made, you will submit the Mentor Application and the Mentor Background Check Form for processing.
